The Data Protection Act
The Data Protection Act (‘The Act’) lays down certain requirements for the protection against unauthorised disclosures of personal information. The Act also gives you certain rights. The information which you give in this form, and any other information obtained during the course of your treatment, is on a strictly confidential basis. This information will be used solely for the purposes of providing osteopathic and/or any related treatment. We will not disclose any personal information which we hold about you outside this practice without your explicit consent, except to the extent we are required or permitted by law.
Data Protection Policy
Jane O'Connor & Associates uses a database to hold the information from the enquiry web form which allows our clients to express their interest in booking an appointment, which we will respond to confirm your booking. This information is entered into a database so technically stored. The Form requires Opt-In Consent for the form to submit and with the option to give Opt-In Consent to add you to our mailing list.
But we will never give/sell or rent this data onto a third party, we may add it to our Jane O'Connor & Associates Mailchimp list upon Opt-In Consent to mailing list. It has the ability to unsubscribe or amend you personal data - we will send out emails 2/4 times a year allowing you to unsubscribe at anytime, we will not be able to re-add you to our list.
Information we will collect to process your enquiry: IP Address, Name, Email, Telephone, How you found us, Other, Message, GDPR Consent, GDPR Consent for mailing list.
Who has access to your personal data: Jane O'Connor & Associates, Mailchimp (Mailing List), Website. We do use Google Analytics on this site.
How long will we hold your personal data: Form Enquiries will be kept for a max of 3 months, if you have consented to be added to the mailing list then until you unsubscribe.
